Patient Financing
Hirshfield Eye Associates will accept Cash, Checks, MC, VISA and AMEX as forms of payment. For surgery that is not covered by insurance we also have Care Credit which can finance your procedure after a simple online or faxed application.
As a convenience to our patients we are also on the panel of many insurance companies including HMO’s or PPO’s. If you are on any of these plans Dr. Hirshfield will accept the fees paid by the insurance company after payment of the proscribed co-payments and deductibles. If a referral is required for a specialist visit it will be the patients obligation to obtain this before the appointment. The following list is not complete as it is always changing.
